While they may not receive a lot of attention, compression garments can be a patient’s best friend after plastic surgery, improving comfort during the early stages of recovery plus helping healing, so your results can look their best. Compression is almost always prescribed after liposuction, tummy tuck surgery, or other surgical body contouring procedures, and it can also be helpful after breast augmentation or breast lift to support your new breast shape.

In fact, compression is a key component of the Rapid Recovery Program for many of our New Jersey plastic surgery patients at the Parker Center, so we decided to share the “whys” and “hows” of compression to help you understand its benefits.

Compression can be a patient’s best friend after cosmetic surgery, helping to minimize swelling, promote faster healing, and support your new contours for optimal results.

What are compression garments?

When patients hear the phrase “compression garment,” they tend to think of Spanx, or the thick, tan stockings for sale in the drugstore to help with varicose veins. While similar in some ways, the compression garments we give our patients to wear after plastic surgery are pretty high-tech, specifically constructed to facilitate healing and enhance results.

Made from strong, synthetic fabrics such as nylon and lycra, post-op compression garments have a graduated, three-dimensional stretch that provides comfortable support and directs subcutaneous fluid (a.k.a. swelling) to be reabsorbed by the body. High-quality garments contain anti-microbial, breathable fabric both to improve comfort and prevent skin irritation.

Compression garments are available for nearly every part of the body, and come in a variety of styles and sizes. Body procedures typically require a garment that extends from below the breasts and fits at the bottom like a leotard or girdle, with openings to allow you to use the bathroom without taking the garment completely off. Following breast augmentation, patients sometimes are asked to wear a surgical compression bra, which looks similar to a high-impact sports bra without underwire. Following neck liposuction, patients are fitted with a compression strap that wraps around the back of the head and underneath the chin to form a “sling.”

Why do I need compression after liposuction (or any other procedure)?

We admit it. Compression garments aren’t the most attractive frocks you’ll find. However, the benefits for your comfort and results are well worth it:

  • Compression minimizes swelling and pain. Having constant pressure on operated tissues helps to prevent excessive fluid from building up, and helps your body absorb fluid that does accumulate.
  • Your results will look great sooner. The less swelling you have in the first place, the less time you’ll need to get rid of it. Without compression helping your body reabsorb fluid, post-op swelling can linger much longer than necessary.
  • Your results will look better overall. Compression helps your skin contract to its new contours, which is especially important for optimal liposuction results. It also helps prevent a wrinkled appearance to skin after any body contouring procedure.

Wearing & caring for your compression garment after surgery

Compression should feel noticeably snug, and but not so tight that it causes discomfort or makes breathing difficult.

Dr. Parker will give you a specific time frame for how long you will need to wear compression. Typically, we advise patients to wear compression 24/7 for the first 1 to 3 weeks, depending on the procedure and extent of surgery. It’s okay to take the garment off to bathe or wash it, but the more you wear compression during your initial recovery, the more effective it will be in facilitating the healing process. After this first phase, expect to wear compression at night for several more weeks to promote healing and protect your scars as you sleep.

Keeping your garment clean will make wearing it more pleasant. Depending on the garment, you may be able to machine wash or hand wash it using mild soap.

How do I know if my compression garment fits correctly?

Compression should feel noticeably snug, and but not so tight that it causes discomfort or makes breathing difficult. Dr. Parker’s nurses will fit you for a garment after surgery that provides the optimal compression while keeping you comfortable. It’s not uncommon for garments to become too loose over time, as swelling goes down. In this case, we’ll have you move to a smaller size.

You may have heard that there’s a new option for fighting cellulite in town. Cellfina™ is a minimally invasive procedure that provides long-lasting improvement of cellulite—and patients are raving about their results.

We recently chatted with Dr. Parker about Cellfina, what makes it stand out from other cellulite procedures, and what patients can realistically expect from their treatment. Here’s what he had to say.

What causes cellulite? And why is it so difficult to treat?

Cellulite is caused by a shortening of fibrous bands, which are like little rubber bands that run from the skin down to the underlying muscles. We’re still unsure of what exactly triggers them to shorten, particularly because cellulite occurs in a wide range of body types, weights, ages, etc. It is likely something hormonally related because cellulite is very common in women—in fact, it’s estimated that around 85% of women have cellulite, whereas it’s very rare in men.

The reason it’s been difficult to treat previously is that there really hasn’t been a consistent methodology that’s yielded good results. Things have come and gone.

Traditionally, how has cellulite been treated? Was it effective?

In the past, cellulite has been treated in various ways, with inconsistent efficacy. Things like Endermologie, where you kind of rolled and vacuumed the skin, had mixed success; there is no long-term improvement because you have to keep doing it. Topical products rarely had a positive impact. Different kinds of laser treatments, including Cellulase, could be uncomfortable without the positive pay-off.

Then you had your surgical options, like subcision. This is essentially the same method as Cellfina, but it wasn’t as effective a technique in achieving long-term results or as comfortable for the patient.

What makes Cellfina different from other options for treating cellulite?

The advantage of Cellfina is that it has standardized and perfected previous subcision techniques. Cellfina tends to be much more uniform with a much higher success rate. Not only is the treatment very simple, it also results in a long-lasting outcome. A trial study was performed where patients were followed over two years and the satisfaction rate after those two years was at 95%. Cellfina is certainly the best long-term treatment for cellulite to date that is currently on the market.

Tell us a little about treatment with Cellfina and why it is so effective.

When we treat somebody with Cellfina, we have them stand up in front of a mirror, and we mark out the areas of cellulite dimples. Because we  most often treat the buttuck and outer thighs, we then have them lie down prone and remark the areas just to ensure they haven’t shifted or moved when the patient was repositioned.

Next, we use a proprietary infusion system whereby we inject local anesthesia into those areas that are being treated. We place what looks like a Petri dish on the patient’s skin and press a button that creates some vacuum pressure to draw up that area of skin, which is right around 3 inches in diameter. When that skin is drawn up at a consistent pressure, all of the skin is being treated the same.

This creates somewhat of a guide, allowing us to uniformly infuse the area with local anesthesia. This is repeated in all the areas to be treated. Using the same “guide” I use for the anesthesia injections, I am able to release the bands by inserting a tiny, proprietary blade 6 millimeters below the skin. Cellfina is very precise in that it allows you to effectively release the band with little chance of recurrence and without doing any damage to the skin.

The precision of this tiny reciprocating blade leaves no scar. It tends to generate some swelling and a little bit of bruising, but it is all very manageable and short-lived. The procedure itself is pretty easy on the patient and the recovery is quick. And, probably most importantly, the results are very good.

Does Cellfina hurt?

With the combination of the grid of anesthesia and Cellfina’s very thin blade, most patients find treatment to be quite comfortable. The first anesthetic injections, before the “numbing” kicks in, are probably the most discomfort a patient will feel throughout the whole treatment. It’s typically a sting and burn sensation, which lessens as more anesthetic is administered.

From that point forward, patients just feel a bit of pressure. The patients are awake for the treatment, though we will occasionally give them something like a Valium or an oral pain medication just to take the edge off. Pain is something that’s variable from one person to the next, so we always “check in” with the patient, making sure they’re still comfortable or taking a break if we need to.

What do your patients find most surprising about their Cellfina treatment?

Probably its relative simplicity and ease—in addition to how successful and long-lasting it is. Typically, if we’re going to have a recurrence with anything in plastic surgery, it’s going to be within a few weeks, a few months. Two years is a great long-term result.

How many treatments are required? Do patients benefit from “maintenance” treatments over time?

When we evaluate a patient, we try to be comprehensive and just give them one treatment. So wherever they have identifiable areas of cellulite, we’ll treat that. With that in mind, most of our patients require only one treatment.

However, it’s important to note that not everyone who has wavy and dimply skin has cellulite. Sometimes, loose skin after weight fluctuation is mistaken for cellulite, which can impact the success of whatever treatment is administered. It’s important for the clinician to differentiate the true dimples of cellulite versus the laxity of skin.

What is recovery like?

Because the blade is so small and the treatment so precise, there are no stitches, only tiny pinholes which heal by themselves without scarring. Through that hole, it is normal to ooze some fluid over the first day or so. Other than that, life can typically resume as normal. Patients can walk around, shower the next day. A little soreness for a day or two is normal, but most patients report that it is very mild.

We often tell our patients to try and schedule the procedure on a Friday and plan to return to work the following Monday, which allows for a few days to recoup.

smiling_girlOne of our newest body shaping options at the Parker Center, CoolSculpting® has been a huge hit, giving us an option for treating patients without surgery or downtime. Now it appears CoolSculpting may have an unexpected side effect for many patients, and it’s a good one: as unwanted fat disappears, skin in the treated areas appears smoother and firmer.

CoolSculpting can be used to treat stubborn fat on the abdomen, hips, flanks, thighs, or back. The apparent skin tightening that comes along with the procedure, however, has come as a pleasant surprise. I’ve seen the effects of CoolSculpting for tighter skin present in two ways:

CoolSculpting appears to preserve skin’s tone & appearance in the treated area.

For optimal results with fat reduction, the skin must be able to contract to fit the new, slimmer body contours. This is why liposuction is discouraged in patients with poorer skin elasticity, as the skin may appear loose after the fat is removed. In contrast, the majority of CoolSculpting patients have seen their skin contract nicely as their full results become apparent. In other words, the reduction in size of a given bulge has not resulted in increased skin laxity, even for patients who have had a fairly dramatic reduction in fat from the treatment.

CoolSculpting may improve skin firmness & elasticity.

A small but consistent number of patients experience an improvement in skin tone and appearance following CoolSculpting. Some have seen a noticeable fold diminish as the skin adopts a smoother, more aesthetically pleasing contour. While only surgery can remove excess skin, the phenomenon is promising news for patients who are bothered by excess fat and a moderate amount of stretched skin, but who are not ready for surgery—for example, post-pregnancy moms who are planning to having more children. For these women, CoolSculpting could provide a cost-effective interim treatment to help them improve their shape while still growing their family.

One of the more prominent theories as to why CoolSculpting may work to tighten skin is that the intense cold triggers the skin to accelerate collagen production. (Collagen is plentiful in youthful skin, but our bodies produce less of it as we age, leading to loss of skin elasticity.) This effect has been dubbed “cryodermadstringo,” or skin tightening via targeted chilling.

Another possibility lies in the gradual nature of fat loss after CoolSculpting. Since the results appear over the course of 3-4 months after treatment, this may simply give the skin time to naturally contract over a slimmer shape.

It’s not uncommon for an unexpected “bonus” benefit of a medical treatment to reveal itself a few years after it comes on the market. I’ve been excited to see patients enjoying this additional benefit from CoolSculpting, but it’s important to note it is not yet FDA approved for this specific use. Formal studies will be conducted to confirm if the skin tightening we’re seeing with our patients is consistent and measurable (as of this writing, such a study is in the initial stages). I look forward to sharing what these studies reveal—in the meantime, CoolSculpting remains an excellent way to lose unwanted body fat without going to extremes.

Recently, the NY Times published an article by Laura Holson titled, “A Little Too Ready for Her Close-Up,” which spurned a media conversation about Hollywood’s sudden about face when it comes to plastic surgery. Casting directors have been sending away actors who have obviously had plastic surgery.

“Frankly, I’m surprised by this conversation,” says Dr. Parker. “I would have thought something like this would have occurred years ago.” It looks like Hollywood is finally catching on to what the Parker Center knew along. For years, the two main goals here have been natural-looking results and a speedy and painless recovery. Dr. Parker calls this “plastic surgery common sense.”

In New Jersey, rhinoplasty, tummy tucks, liposuction, facelifts, and breast augmentations are the most popular
procedures. Dr. Parker uses computer imaging, patient education, an experienced team on registered nurses and board certified anesthesiologists- as well as his Rapid Recovery program- to achieve the above principles.

With procedures like breast augmentation, New Jersey patients are fully educated and sessions are performed preoperatively with the staff to help achieve results and a breast size with which they are comfortable. We often hear from our patients about these sessions and how helpful and comforting they are.

It would be disingenuous to say that Hollywood is completely against plastic surgery. However, as mentioned in the NY Times article, everything in moderation is ok. If casting directors can’t tell if an actor has had plastic surgery then it’s no big deal. It’s only obvious, over-done plastic surgery that catches their negative attention.

Now, Dr. Parker cannot attest to what is popular on the West Coast. We can speak to is that his New Jersey plastic surgery patients want a more natural, subtle appearance. They want to look like a better- not freakish- version of themselves.

New Jersey plastic surgery procedures when she came to see us in May 2009 for a liposuction consultation. She had lipo of the abdomen 15 years ago with a different surgeon which explains some of her apprehension before her first procedure at The Parker Center.

“I was in agony after my first liposuction of the abdomen,” she says. “And I think I had one post-op follow-up and never saw my surgeon again.”

When asked how her experience 15 years ago compared with her experience with Dr. Parker, she says, “It was completely different. I never even filled my pain medication prescription.”

When she was looking for a new surgeon for liposuction in New Jersey, she did her homework. She had heard of Dr. Parker through the doctor’s office where she works and had heard good things about him. Once she Googled him, she was “sold just by looking at his Website.”

28 year-old model, Mesha Seville, wanted to make some minor improvements to her nose by narrowing the tip and nostrils just a bit.

“I liked my nose, it just needed a few tweaks,” she says. “I wanted a little softer look to my face.”

So, she scheduled an appointment at The Parker Center for Plastic Surgery in New Jersey. Rhinoplasty wasn’t the first surgery New Jersey plastic surgeon, Dr. Paul M. Parker, performed on her. “I dieted, exercised and nothing happened,” she says. “So I asked around for referrals for plastic surgeons in NJ. “Mommy Makeover” is an umbrella term for a series of procedures to correct problems caused by pregnancy, and it has taken the cosmetic surgery field by storm. “The ravages of pregnancy can take on a variety of forms,” says New Jersey plastic surgeon , Dr. Paul M. The consultation process is integral in determining what the right procedure would be for each individual mom. New Jersey breast enhancement, tummy tuck and liposuction are the three most common options for makeovers.

Lipoabdominoplasty is a fairly new procedure and New Jersey plastic surgeon, Dr. Paul M. Parker is one of the few surgeons in the region who perform it. It is the innovative approach and quick recovery that makes this procedure so attractive to Dr. Parker’s patients who started out seeking a traditional tummy tuck.

One patient, however, knew exactly what she was looking for when she found Dr. Parker’s Web site. Linda is a soldier in the United States Army, a mom and stationed overseas in Great Britain. When asked in an e-mail interview why she was specifically looking for a surgeon who performed lipoabdominoplasty, she replied, “Looking at other before and after pictures, there seemed to be better results [with this procedure].” The quick recovery was also an important factor in her decision process.

First, Dr. Parker performs liposuction on the entire abdominal area. Then, as he removes excess skin and fatty tissue, he is able to preserve more nerves, blood vessels and lymphatic vessels than in a traditional tummy tuck.
